Hospital Passports

Health Passports can be useful for people with learning disabilities when they access hospital, or any healthcare setting.   They are brought with the person to the healthcare setting, and contain information which is useful for health professionals to know about the person with a learning disability.   Health Passports help to ensure that the person has their needs met when accessing healthcare, and they assist healthcare professionals to make reasonable adjustments when needed.
