The Rapid Intervention Team (RIT) is integrated with Somerset’s Learning Disabilities Specialist Health Teams.

RIT supports the Trust’s commitment to Somerset’s Transforming Care Programme, and their role includes:

  • county wide crisis support to reduce the risk of complex placement breakdown or inpatient hospital admission
  • clinical oversight of the Dynamic Support Register and Blue Light Meetings
  • partnership working with health and social care colleagues including the REACT team
  • liaison with emergency service and specialist forensic health colleagues in relation to service users with learning disabilities who are at risk of offending
  • health care coordination/key working for people with complex health and social care needs, including patients with learning disabilities and/or autism who have been detained under the Mental Health Act (including out of area hospital patients)

Referral Information:

RIT works with individuals aged 18 years and over with a diagnosed Learning Disability and/or Autism who are at risk of complex placement breakdown and/or a hospital admission.

  1. Referrals for people with learning disabilities are made through Somerset’s Learning Disabilities Specialist Health Service via the following email and will be discussed in daily single point of access meetings: CTALDHealthReferrals@SomersetFT.nhs.uk
  2. Referrals for people with Autism are made via the Somerset Integrated Care Board’s Commissioning Managers for Learning Disability and Autism.
