What is a health coach?
Health coaches have transformational conversations to help individuals identify what they want to achieve, and work toward achieving it themselves. Using a personalised conversational approach they help you identify what is important to you.
What is a health coach?
A health coach can help you to gain the knowledge, skills, tools and confidence to take
responsibility of your lifestyle choices and provide personalised care. They use broad knowledge
of health and wellness to help you navigate a variety of health concerns and empower you to
take part and be an active participant in your care and behaviour change.
Health coaches will spend time with you discussing your health and lifestyle. By listening to what
really matters most to you the team will help you to identify realistic health goals and develop
your confidence so you can successfully manage your own health and wellness.
How do I see a health coach?
You can self refer for health coaching by completing the form on the referral pages of this website:
Or by calling us on 07917 251 587 and leaving a
message.
You will then receive a call to discuss your referral. Following this you will be contacted by your allocated health coach to arrange your first session.
How can a health coach help me?
Health coaches have transformational conversations that help you identify what is important to
you.
They help you make a plan and identify any barriers that may occur in achieving this.
They will help you make changes in your daily
routine by:
- motivating and empowering you to identify achievable and realistic goals
- help you find relevant information
- suggest interventions specifically for your needs
- find local peer-support
The coaching sessions are one to one and can be by video call, telephone, or face to face within
a community setting, for example, a local library, community centre, leisure centre or a walk in
the park, whichever makes you feel at ease.

If you are not registered with any of the above surgeries we are sorry to say we cannot accept a referral for you.
Most surgeries have their own health coaches and if you contact your surgery directly they may be able to refer you to the appropriate service or tell you how to access them.
