The current coaching service delivered by Somerset NHS Foundation Trust for Taunton Central and Tone Valley Primary care Networks (PCNs) will come to an end in early December 2026.
Referrals should continue to be made through the usual pathways unless advised otherwise.
The PCNs are currently exploring future provision and are working to identify a new provider to help patients manage their health and wellbeing. Further information will be shared as soon as arrangements have been confirmed. Patients who are currently receiving support from a health coach will continue to receive care as normal until the service transition takes place.

Taunton Deane West PCN are also providing an alternative provider and patients can access this service by contacting either Wellington Medical Centre or Lister House surgery directly.
