Diabetes

From the age of 16 you will be invited to appointments with your current diabetes team and the new diabetes team that you will be working with.

We do this as it gives everyone time to get to know each other and feel comfortable before the move to adult diabetes service.

You will be moved over to the adult services on or shortly after your 18th birthday.

You will have a hospital consultant, dietitian and nurses to support you as usual. You might also see a nurse or GP at your local doctor’s surgery.

Your appointments don’t always have to be at hospital. Please discuss with us if you would like your GP to take the lead in your care.

If you move out of the area (to go to university for instance), talk to us about changing to a new local diabetes team.

Sick day rules

It is important to know your sick day rules and how to manage ketones. Please make sure you talk these through with your diabetes team.

Pregnancy

It is important to avoid unplanned pregnancy. If you are planning on starting a family, please ask your GP or diabetes team for advice.

Patients moving to a neighbouring hospital for their adult care may find the transition process is different. Please discuss this with those currently providing your care.
