Before you are 18 you will be invited to meet your adult rehabilitation consultant and adult neuro physiotherapist along with the staff from the children’s department. This is so that everyone can get to know each other before your move to adult services.
In addition we can discuss your goals and what support you might need. We will be able to write a clear summary letter for you and your GP.
If it has been possible we will organise follow up appointments in adult services. If this has not been possible the GP will be able to make the referral.
A health passport will help all medical staff know how to support you best, so writing one is important.
If you have a learning disability you will be entitled to an Annual Health Check from the age of 14 at your GP surgery. The adult learning disability service should be there to help.
Some of your health needs will need long term support. Other health problems will be looked after by your GP.
You will be given appointments by physiotherapist rehabilitation team.
Patients moving to a neighbouring hospital for their adult care may find the transition process is different. Please discuss this with those currently providing your care.
Please do not stop or change any of your medications unless told to by one of your medical team.
