From the age of 14 you and the adults who support you should start talking about:
whether you will need medical support in the future
who will give you any medical support you need
We given you some general information about what your transition plan might look like for each medical conditions.
We encourage you to discuss these transitions pathways with your medical professionals regularly.
Use the transition planning paperwork to help you prepare for your move from children’s healthcare. You can email transition@somerset.nhs.uk to receive a copy for you to use.
