From the age of 14 the paediatric asthma staff will start talking to you about the adult asthma service. You and your family will be invited to visit the adult inpatient and outpatient area. Your paediatric nurse will introduce you to your adult asthma nurse.
You will have some appointments where both your current asthma staff and future adult asthma staff will work together to make the move go smoothly. The first of these joint appointments will be in children’s outpatients and then they will be in the Somerset Lung Lab. If you also have support from Bristol Royal Hospital for Children, you may move to the adult team at Bristol.
There are lots of helpful resources produced by Asthma + Lung UK.
Moving On Asthma has been developed by the Respiratory Research Team at Sheffield Children’s NHS Foundation Trust.
Make sure you have a personalised asthma action plan. You can make one on the Asthma + Lung UK website.
Patients moving to a neighbouring hospital for their adult care may find the transition process is different. Please discuss this with those currently providing your care.
