Complex health needs

You may have very complex health needs and have specialist support called Children’s Continuing Care (CCC) support on top of other health services.

If so, from around the age of 14 discussions will start about moving to adult services for your healthcare needs. These will include you, your family and the Integrated Care Board (ICB).

At the age of 16 to 17 years of age the CCC team will start talking with Somerset Adult Social Care staff and staff in the Adult Continuing Healthcare (CHC).

An application for CHC support will be completed by the age of 17 and a half. The application does not include past support or possible future needs. An Education & Health Care Plan (EHCP) may be used in the application.

Your voice and views are important. Some young people may not have the ability to understand information and make decisions about their lives. For these young people a mental capacity assessment may be discussed as they become 18 years of age.

  • If you qualify for Continuing Healthcare (CHC), a transition nurse assessor will stay in contact to support a smooth transition. Adult Social Care would not be involved.
  • If you do not qualify for Continuing Healthcare (CHC), Somerset Adult Social Care will assess the your care and support needs.

Patients moving to a neighbouring hospital for their adult care may find the transition process is different. Please discuss this with those currently providing your care.
