Mental health
If you are working with:
- Children and Adolescent Mental Services (CAMHS)
- Children’s Eating Disorder Service (CEDS)
- mental health inpatient unit
Before your 18th birthday, a decision needs to be made about your future care. You will have a keyworker who will talk with other healthcare professionals at a transition meeting about what you have achieved and how your recovery is going.
Next steps may be a planned exit from the service. This would be the best possible outcome and often happens as you form new relationships and take part in activities that come with being an adult. A planned exit would include you knowing how to get support if needed in the future.
Alternatively a referral to an adult service to support continued recovery may be made. A service called Connect 18 may support in this referral or work directly with the you.

Connect 18
They work directly with young people who may have experienced trauma and have Complex Emotional Needs (CEN) who want to actively work towards their recovery.
Like other health services their support is structured and time limited.
The aims of the service include:
- supporting your safety
- preventing and reducing hospital admissions
- improving your relationships across all parts of your lives and helping you to become an
independent adult.
Open Mental Health
Anyone can contact this service about their own mental health or if they have any concerns about another adult at anytime 24/7 via telephone 01823 276 892 or Freephone 0800 138 1692.
Talking Therapies
Anyone over the age of 16 who has anxiety and/or depression can complete an online form to request support. The form and more information is available on the Talking Therapies webpage.
Your current healthcare team can make contact and agree a support plan. Like other health services the support will be time limited and could include virtual and/or face to face appointments. This service is not suitable for people with thoughts of harming themselves or others.
First Response Service
This service works with anyone who is in a mental health crisis such as feeling overwhelmed and struggling to cope. They can be contacted anytime 24/7 on 0300 124 5016 or Freephone 0800 138 1692 or via Mindline 01823 276892. More information is available on the First Response Service webpage.
There are some other services within the Community Mental Health (SMH). An NHS professional needs to make the referral to any of these services.
Dual Diagnosis Service
They help people who have both severe and long-term mental health difficulties and are misusing substances in a harmful way. For more information see our Community Mental Health Service webpage.
Eating Disorder Service for Adults (EDSA)
They help people with moderate to severe eating disorders. For other support available see the websites for Beat Eating Disorders and Somerset & West Eating Disorders Association (SWEDA).
Service for Early Intervention Psychosis (STEP)
They work with people aged 14 to 35 who are having or are at high risk of developing psychosis. Psychosis is when someone has some loss of contact with reality. For more information see our Early Intervention Psychosis webpage.

The Somerset Big Tent website lists lots of different organisations in different parts of somerset they maybe helpful to young people aged 5 to 25 and their parents/carers. The organisation is approved by professionals.
