We welcome you to come along and discover how you can manage your musculoskeletal condition, with support from the physiotherapy team, other professions and local community providers, all under one roof.

You will be given your own Patient Passport, to guide you through the day, while you access a number of different sessions, which could include:

  • ‘What Matters to You’ conversation
  • advice and guidance
  • rehabilitation
  • 1:1 physiotherapy assessment (if required)
  • local community services

What is a community appointment day and other questions...

What is a Community Appointment Day?

This is your physiotherapy appointment and is an opportunity for you to discuss ‘what matters to you’ with MSK therapists and other specialist community services.

This is a new way of working for our service as we look to increase our community engagement in the musculoskeletal services we offer and there will be opportunity to comment on your experience at the end of your time with us at the community centre.

How long will I be there?

Your appointment time will have been provided to you on the letter you received inviting you to attend you MSK Physiotherapy appointment.

You will have as long as you need, there is no time limit on these appointments. Each person’s appointment will be different but people typically spend 60 to 120 minutes at the event.

What will it look like?

The event is being held in a number of different rooms in the community centre. There is one entrance and exit to the event which will be clearly signposted. You will be directed to the ‘What Matters to You’ room, for your initial clinical conversation. From there you will be guided around the centre to the section(s) you need to see.

In addition, there will be community services in the main hall to allow you to quickly gain ongoing support for your health needs as required.

Throughout your appointment you will complete an individual patient passport.  For the majority of people this will be an electronic document on your phone or tablet that will be enabled when you arrive for your appointment (assistance will be on hand to help you with this as required).

We ask that you bring your device with you, fully charged to allow you to access this.  If you are unable to access an electronic device on the day this can be completed on a paper copy.

There will be a large number of people attending throughout the day, so it may well feel loud at busy and the event. If you have accessibility or sensory needs, please call us to discuss if the day is right for you.

How to prepare for your appointment

  • Consider why you were referred to the MSK Physiotherapy service.
  • Consider ‘What Matters to You’ about your condition.
  • Think about what you have done about your condition already.
  • Think about what you would like to do about your condition or what support you think you need.
  • Charge and bring your mobile phone or tablet.
